Every one can experience ventricular ectopic beats, particularly after vigorous physical activity or excessive psychological stress. Hormonal changes are not negligable, also. My opinion is that you are in good control with propranolol, which also acts on stress reduction. However, I would recommend to switch it to atenolol, which provides better control on the heart rate, resuces heart workload and prevents arrhythmias. I would stress that ventricular ectopic beats are benign phenomena and could happen to anyone.
If these phenomena are affecting on your qiality of work, life or everyday activites, consult a paychiatris regarding panic attacks, and proper treatment with sedatives should be installed, to surpress or control bursts of panic or other forma of anxiety.

I have a loop recorder , cardiac MRI 6 years ago normal heart structure ..
It had been ruining my life as I quit my favourite nursing job and stopped riding my horse scared to go anywhere or do anything
I have started work again and slowly riding my horse . Today however quite strenuous riding only for 15 minutes lots of ectopic and SOB
I'm scared I'm going to die if I exercise I am unfit
And the most concerning during big attacks is the sensation I'm going to loose control of my bladder /bowels .. Is this associated with cardiac issues

My opinion is that the ectopic heart beats are secondary to your panic attacks, meaning that they produce these beats. You see, every time heart experiences hyperdinamic actions, it is normal and with high probability to produce ectopic beats. Panic attacks do not have clear-cut known causes, but they have known triggers such as being in crowded places, before performing in front of public etc. It is understandable and expected that you have a normal heart structure and, in your case, the ectopic beats are not related to the heart structure problems, but merely to the general anxiety disorder and panic attacks. That is why I recommendes you to see the issue with your psychiatrist, to see the possibility of combining a sedative from benzodiazepines with a mood-controller from anti-psychotic drugs. I would not recommend you to stop physical activity, as it is a very beneficial component, physically and psychologically, in our everyday life. Also, I will say it again, consult your cardiologist, as I would recommend you to be switched to atenolol, if I would be your treating doctor.

Various anxiety patients perceive their episodes as "something ia going to happen to them". I have met many patients with this disorder. Some have asked me "am I going to die?", "if I collapse, will anyone help me?", even the concern you are addressing. I will reassure you that that incident will not happen from ectopic heart beats. There are other certain neurological diseases, which can be accompanied with loss of bowel and/or bladder control. If you had those, they would be diagnosed in your childhood. Not a single episode is registered, as we speak, that during these symptoms, you could lose bowel and/or bladder control.
