This looks like a regular bruise caused by an abrasion and rub. What you described as a rash looks like a typical bruise part.
Please note that during injury (even if you can not recall it, this seems the most probable reason) small blood vessels (veins mostly) get injured. This causes blood appearance outside the blood vessels. In some areas of the soft tissues there will be more blood collected, in others there will be less.
This collected blood gets removed by special blood cells. Now from the areas where there was less blood, it will disappear quicker. From those areas where there was more blood it will be disappearing longer. The rash seems to be caused by such process.
And as the bruise can be present even for 4 weeks, this seems a regular image of a bruise. No need to do anything with it. Just let the nature to do its job.
