Suggest Treatment For Blockage In Left Fallopian Tube And TB Infection In Uterus

Question: My wife aged 34 years and have a 5 years Baby Boy. We were trying for second baby but not successes so HSG and MT test of my wife was done and found that her left fallopian tube was blocked and TB infection in Uterus. So Doctor advised for AKT4 & 3 for 9 months. Accordingly she had taken AKT4 from 18th April to 18th October (6 Months) and from 19th October to till date (2 months) she had taken Rcinex 600mg & combutol 1000mg. Means 8 months of medications are completed.
Doctor also advised that up to 6 month not to take chance for second baby. So in last month only we have taken chance and now we came to know that she had pregnancy of 6 weeks (1.5 month). Now as per TVS scanning, the baby is in Uterus. Now we want to know that:-
(1) whether above medicines (Rcinex 600mg & combutol 1000mg) can be continued.
(2) If yes, whether it affects the Baby
(3) If No, whether 8 months medication is sufficient or chance to not completely cure TB infection and it affect baby.
Please reply in detail and point wise.

It is great to hear that your wife has been able to conceive.

A treatment for a minimum of 6 months is advised but it needs to be checked thereafter. Since she has already completed 6 months of treatment, I would insist on getting her tested first before deciding if the medicine needs to be continued further. In most cases it is successful in fully curing the infection.

Let me mention that untreated tuberculosis in pregnancy poses a significant threat to the mother, fetus and family and hence treatment of the disease would be a priority if the results indicate that further treatment is required even though the medicines could indeed have some effect on the pregnancy. All I could say is that the medicines that you have mentioned are relatively safe and can be used without worrying about adverse effect on the fetus. The medicine strepomycin is to be avoided if further treatment is required.

Thanks for writing back.
TB culture, and PCR tests targeting devR (Rv3133c) and IS6110 gene sequences are confirmatory tests for extra pulmonary tuberculosis. But before going for these tests please talk to your treating physician. In normal cases extapulmonary tuberculosis needs 6 months of intensive treatment followed by another 4 months of treatment with rifampicin and izoniazid. Whether the doctor thinks it necessary to complete the entire regimen first, needs to be discussed first.
