Question: Periodically experience blood and bloodclots in urine. Have received external beam radiation in 1990 for prostate cancer: suffer from radiation cystitis; have had bladder cauterization; kidney stones; am on coumadine; Am at a loss at cause of bleeding; comes and goes; bursting blood vessels?

Hello and welcome.

I have read your history carefully.

Acute symptoms of radiation injury to the bladder are self-limiting and generally respond to symptomatic therapy, such as anticholinergic medications and analgesics. Severe complications of radiation injuries are difficult to manage because they tend to be chronic or recurrent and are occasionally refractory to therapy.

Blood in urine (hematuria) needs proper work up before any treatment is initiated.
Unless there is a history of trauma vascular injury is unlikely as you had concern for rupture or bursting of a vessel.

Firstly, I would advise you to stop the Coumadin and get your INR checked via your primary healthcare provider to see if its supra therapeutic or not.

Secondly , You need to get the following tests done in consult with your doctor to have a better idea of the precipitating cause for your symptoms

CBC
URINE R/E
URINE CULTURE AND SENSITIVITY
BUN, CREATININE (RENAL FUNCTION TESTS)
BLADDER AND RENAL ULTRASOUND

INR OK…Is Checked regularly….. Also, whenever I have bleeding I discontinue Coumadin a few days...

Even if INR is in range , i.e between 2 and 3 , still the investigations that I mentioned previously should be done in consult with your doctor as there are multiple causes of blood in urine.

Treatment can only be initiated when the cause is isolated.
