Question: Hi Doctor, my wife (ago 40) was just diagnosed breast cancer, with BI-RADS 4c (by ultrasound) and BI-RADS 3 (by molybdenum target method). I want to know does she has to take Mastectomy surgery instead of Lumpectomy?

Brief Answer:
may not need mastectomy

Detailed Answer:
Hi

Thanks for your query.

I assume that biopsy has already been done and cancer confirmed. From the USG film it looks like a small tumor. In that case lumpectomy is good enough. The lymph nodes in the armpit also need to be either taken out or tested by sentinel node biopsy.

However, if the surgeon feels that the size of the tumor is large relative to the size of the breast then he can advise chemotherapy to shrink the tumor and then do the lumpectomy.

Hope this helps. I will be available for follow up.

Actually biopsy not done yet, cancer not confirmed yet.
I did some translation of the ultrasound result, please see the uploaded image file ( I add some translation to the file and uploaded it again).
Thank you Dr. Indranil

what the doctors here said is: they need to cut out the tumor and then check it whether it's good or bad, they haven't confirm any yet.

Brief Answer:
would like to have biopsy confirmation first

Detailed Answer:
Thanks for followup.

It is always to better to confirm the diagnosis before going for surgery, either by a fine needle aspiration cytology or needle biopsy.

If this is not feasible, then doing the lumpectomy in OT followed by frozen section biopsy (during the surgery itself) and then taking out the lymph nodes if cancer is confirmed, can also be done.
