Brief Answer:
Prognosis is good if patient follows proper treatment.

Detailed Answer:
Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
COPD(chronic obstructive Pulmonary disease) is progressive disease.
If you follow GOLD guidelines for treatment of COPD then prognosis is good.
Usual 5 year survival rate is 70-80%.
Following are the treatment guidelines.
1. Quit smoking as soon as possible. This is the most important step.
2. Take regular inhaled bronchodilators and inhaled corticosteroid. Start long duration agents.
3. Enroll yourself in Pulmonary Rehabilitation center where chest physiotherapy and deep breathing exercises are done. These will improve lung function and thus decrease the symptoms.
4. Get done adult respiratory vaccination with pneomococcal and Influenza vaccines to prevent recurrent respiratory tract infection.
So consult pulmonologist and discuss all these.
Hope I have solved your query.
I will be happy to help you further.
Wish you good health.
Thanks.
