Brief Answer:
These could be genital warts by HPV infection.

Detailed Answer:
Hello! I have been through your question.

Please first could you send me an attached picture of these bumps and the sexual history of your brother in order to define better the diagnosis?

Related your brother symptoms I think that these bumps could be as a result of a HPV( human papillomavirus) infection.
You should know that usually all sexually active people will get HPV at some time in their life and the most men who get HPV infection never develop symptoms and the infection usually goes away completely by itself.
But if HPV does not go away, it can cause genital warts (bumps like your brother) that usually appear as a small bump or group of bumps in the genital area around the penis(like your brother) or the anus and may be small or large, raised or flat, or shaped like a cauliflower.

I hope my answer helps you.
Feel free to ask me further.
Regards.
