Suggest Treatment For Carcinoid Tumor

Question: I had a octetide scan for the carcinoid tumor it showed activity in my liver but not a size in the tumor. He also did the H15aa urine test that was elevated. I have flushing and lesions on my nose and cheeks. I am extremely tired and feel weak. I feel like I have asthma I never had before. I had a carcinoid tumor on my appendix 9 years ago that was removed. They want to do a liver biopsy. What kind of treatment should I have?

Thank you for giving me opportunity to share my opinion regarding the disease condition of yours. I can understand the depth of anxiety and tension you are facing since long time after going through you history.

Anyway I would like to throw some light on the present scenario of the disease.
This carcinoid tumor is such a tumor which most of the time is diagnosed after appendectomy as it is commonly found to be affecting appendix either in the tip of it or in the base of appendix.

This tumor can also grow in any part of the digestive system (mouth to anus), in liver, in lungs, in gland, ovary,etc.

Problem with this tumour is they secrete some hormone or substance which affects other system of the body, resulting into various unwanted effects like breathlessness, pain, loose motion, constipation, uneasiness, flushing, weakness, tiredness, asthmatic like attack,various bodily discomfort, etc.

As I told if it is in the tip of appendix then simple appendectomy is sufficient, but if it is in base of appendix then total right half of the intestine have to be removed to cure the disease in Toto.

If it is found to be in liver than that part of the liver has to be removed.

So presently as it is seen to be present in liver as per octreotide scan you mentioned, so it has to be confirmed by tissue biopsy, which is must.

There are various medicines also available to temporarily stop the activity, and effect which the tumour is doing by secreting hormones and various substances.

The medicines are the following:-
1. Octreotide,
2. Pasariotide,(2nd generation)
3. Bevacizumab,
4. Sumanitinib,
5. Interfiron,
6. Cyprodeptadin.etc.

The treatment also depends upon the size of tumour, situation of tumour, number of tumour in body, whether in operable condition or not,etc

As detailed history of yours is not available with me as mentioned above, I would like to advice you to get the biopsy done to confirm the diagnosis, followed by detailed discussion by your treating doctor regarding the treatment option available in that hospital.

If the tumour is small and symptoms could be controlled by medicines then you can go ahead with medicines, but if the tumour is big and is operable then you can opt for surgery.

If possible kindly keep me updating about your various other investigation reports then it will be easy for me to guide you further more.

Thank you so much for spending the time to look into this for me. I have one more question. I have pain that comes from under my right ribs that goes to my back. It hurts worse when I eat. Would this be from the liver? I have no gallbladder.

As you have asked whether the pain is from the liver or not?

Well as per my clinical experience goes since your liver is involved by the carcinoid tumour, there is 70-80% chances of pain coming from the liver.

Besides that there are other things also which mat give pain like inter-coastal muscle or rib itself or may be the lung or pleura,may be intestine (large intestine) kidney (right), etc.

Hence I advice you to, do discuss this matter also with your treating doctor and get it confirmed .
