Suggest Treatment For Cervical Incompetence

Question: Sir,
I am 18 weeks 3 days old pregnant lady.
On ultrasound, it was discovered that length of my cervix is only 2.9 cms.
Is this a cause to worry?
My gynecologist became too serious after looking at my ultrasound report and advised immediate bed rest.
I had to travel by air the duration of my travel was 04 hrs. but, now I am postponing my trip.Kindly advise:
1) Cervix length of 2.9 cms. is a cause to worry?
2) Air travel in 19th week is really harmful for me & baby?

Thanks for posting your query.

Normally, the length of the cervix is about 30 mm. Cervical incompetence is diagnosed if the length of the cervix is less than 25 mm. before or at 24 weeks of gestation. This is in addition to several other features that include painless cervical dilatation. Previous medical history of procedures on the cervix and previous abortions or preterm births is significant in making the diagnosis.

In the current situation, I feel that there is no particular need to worry if you do not have any significant past history suggestive of an incompetence. Only, regular monitoring is needed and cervical encirclage, which, ideally is done between 14-16 weeks of pregnancy may be considered if there seems an imminent risk of preterm labor.

Travel, by any mode can cause some strain on any pregnancy. If at all needed, the best way to travel is by train. In your case, I would ask you to put off travel unless absolutely necessary, at least until things sort out.

Also, please do not exert, take adequate bed rest and avoid sexual intercourse. It would also help to lie with the foot end elevated.

Thanks for answering my query.
Actually, just before pregnancy, I suffered from E.coli and undergone medication. At present my report reveals 10000 colonies/ml grown.
My hemoglobin Hb, Blood, (Photometric) is 9.20 g/dl.
Please advice that whether E.coli prior to pregnancy and whether hemoglobin level below benchmark will have any adverse impact and how can hemoglobin be increased.(my hemoglobin level is poor since my birth)

Bacterial vaginitis in pregnancy can pose a risk if it persists and is severe. There is a possibility of miscarriage, infection and preterm birth.

Also, it should be discriminated if there is bacterial vaginitis or aerobic vaginosis, since the former is harmful while the latter is mostly harmless. While the former consists mainly of anerobic flora, aerobic organisms like E.Coli are seen in the latter.

Treatment is usually by antibiotics and probiotics. Antibiotics may be used as and when needed and by oral and vaginal routes.

Though your hemoglobin levels are slightly lower, you need not worry. With good hematinic supplements and proper diet you can recover well and in time. Please increase the intake of green leafy vegetables, milk, dal, and if you are a non-vegetarian, please take a boiled egg everyday. Also, increase the intake of iron rich food like dry fruits, jaggery, liver etc.
