
Suggest Treatment For Chest Pain When Diagnosed With GERD And Hypertension

My name is xxxxx and I am 25 years old. My weight is 80 KG and height is 5 feet 11 inch. Since the past couple of months I have been experiencing breathlessness and
pain in the chest area when I climb stairs or run.
The pain sometimes is sharp and can be pointed to upper left area of chest. Sometimes its more diffused around center of the chest and I feel pressure around the
chest region. The pain or discomfort reduces and disappers after i take rest. Sometime the pain also starts abruptly when i am at rest mostly at night, during which there
is an increase in heart beat and it reduces and goes away on its own sometimes follwed by burping. Sometimes the pain starts due a lack of sleep late at night.
I have slightly high BP @ 130/90 and I think this is because high BP runs in our family.I also have heatburn, acidity and my stomach bloats beacuse of gas , could this be
the reason or is it beacuse of my weight and possible build up of bad fat in blood vessels. I am confused as to what is causing the chest pain.
I fear that I may be suffering from a heart dieases like atherosclerosis.
I have a history of GERD and hence had to take Pan 20 and sometimes pan 40 tablets in the past.
I also guess that i suffer from gynecomastia, this could also be psedo gynecomastia due to my weight but i have not checked that up with a doc.
Please suggest me what diagnostic tests are required to reach the root cause of the problem? Also please feel free to reply back for any missing information.
The symptoms still suggest GERD as the cause

I am Dr. Mody and I would be addressing your concern.
I went through the history you have provided in detail and I thank you for the same .
Your weight of 80 for the height is not very bad and , it's just 3- 4 kilos away from the goal , hence per say the weight should not be the reason for symptoms or even gynaecomastia.
The symptoms you describe and considering your age still points towards GERD as the probable cause of symptoms.
Why not the heart ?
The type of pain . Heart pain generally can not be localized.
You are a young healthy man and unless you smoke , they risk of having atherosclerotic obstructive lesion is rare .
The burping , suggests GERD..
Pain occurring at rest and night again suggests GERD or muscular as the cause .
The symptoms occurring when you climb stairs particularly can be because you are not used to exercise .
The blood pressure is just border line normal .
You may cut salt in your diet and consider aerobic exercise daily for at least 45 mins , which is will help you decrease weight ,reduce chances of future heart events and decrease your pressures too .
Instead of pan 20 .
A trial of Sompraz D 40 mg in morning before breakfast for at least 3 weeks .
Avoid tea , coffee, chocolates, aerated drinks, spicy and oily food and take smaller meals for complete healing.
As such based on clinical history I wouldn't keep heart as my 1st differential, however you may Consider getting an ECG and echo done for heart and lipid profile to know your status of cholesterol and triglycerides considering family risk .
