Suggest Treatment For Chronic Back Pain And Pulsating Sensations In The Testicles

Question: Hello doctor,

I have gone through many of the question you answered and I have noticed a really high quality of care and understanding. I hope you can help me with my issue.

I have a chronic complaint for the last 30 years, I have gone to many local doctors over the years who all of them have not fully understood my issue and prescribed general pain management medications that brought no relief.

Complaint description:

sensation of pulling, some sort of contraction and throbbing (pulsation) in the region of the testicles internally (can't point it), it comes when I lay on bed preparing to sleep for a duration of 1 to 2 hours, it doesn't cause pain but it makes me VERY uncomfortable and prevents me from sleeping for the duration.

Additional information:

One hard to explain characteristic regarding this condition is that when I am injured, let's say for example I have a wound on my finger when the condition starts the pain in my finger increases dramatically. I was able to live with this condition from my youth years up to recently I started to have a chronic upper back pain that gets unbearable when the condition starts. Few months ago I was given some muscle relaxant injection for my chronic back pain and it completely stopped this condition for a day. However, I was told that such medication is not made to be used for long-term treatment. No trauma what so ever to the region. Finally, all the doctors I visited in the past haven't noticed anything abnormal during physical exam.

I hope everything regarding my complaint is clear. if there is any additional information needed or anything is not fully addressed please do not hesitate to ask.

I am Dr Saad Sultan and will be answering your query... I have gone through your problem and fully understood your concerns....
As far as pulling sensation is concerned, in my opinion it should be examined clinically. Sometimes small inguinal hernia , varicocele, hydrocele presents with this. It should be examined by the general surgeon so that it can be properly dealt. Painkillers is not an answer to this condition, its just a temporary measure...
You may need to get the ultrasound scan of the abdomen and pelvis along with the scrotal ultrasound to differentiate the pathology...
Chronic back pain needs to be investigated. In most of cases it is musculoskeletal in origin. It may be examined by orthopedic surgeon . you may need to get an x ray of the of the spines affected . MRI is next level investigation if x ray and clinical examination demands about it..
Meanwhile use acetaminophen to relieve pain.
