body requirements /tolerance of medicines change with age
Detailed Answer:
Hi
Your treating doctor may have prescribed current dosage and medication as per situation with your health status now- as a person grows in age, the body requirements, tolerance, systemic (body organs functions) responses become slower and depending on pulmonary function tests, liver and kidney function tests doses are modified time to time. He may have assessed both clinically or by tests about these functions and may have modified the dose to save you from other risks with slower metabolism. BMI is a rough index for body metabolism and function based on weight and age and height. I am sure there must be changes with BMI or basal metabolic rate (BMR) to reduce medications. The opioids taken by you is well over 10 years now and a sort of dependency develops in course of time, so you feel restless, but as with all narcotics, respiratory suppression in older age is a possibility- so faith and trust in your doctor is necessary. You will have same benefits of previous dose as per current assimilation/absorption of drug
For rotator cuff tear, your doctor may refer to shoulder surgeon if thinks necesary for any surgical repair required if he asseses so.you may use a broad arm sling for taking off weight of arm. Some physical therapist may do some passive/active shoulder exercise as per recommendation by your examining doctor.
I would consider adding anti-inflammatory medicines for pains as well and local applications of muscle relaxant gels/anti-inflammatory gels. You may see your doctor ad get these.
Best wishes,
Regards,
