Brief Answer:
Painful Varicocele can be best treated by Surgery

Detailed Answer:
Hi.
Nice to see you again after March 2014.

Read and understood your history, that you have a symptomatic Varicocele.
You have got the consistent pain and discomfort.
OTC (over The Counter) medicines may not work at all as the cause of pain is different in Varicocele.
It is good that you have an appointment and your Doctor surely will prescribe proper medications.
He may prescribe you medicines by his experience.
But the most important thing is that the painful varicocele may be treated by surgery alone.

He may prescribe you some medicines like Aesculus hippocastanum (Horse chestnut) seed extract/Diosmin if he believes in its effects.

In addition you can try the following:
Scrotal support just to lift the scrotum.
Cold water bath.
Lying down (bed-rest) whenever possible.
Avoid long standing
Loose clothing
Avoid hot and humid atmosphere.

I hope this answer helps you, please feel free to ask for further relevant questions if you need to or if you feel that there is gap-of-communication.
