Question: II have fever,chills and sweats. I have some coughing and constant headache. My appetite is deceased.

Hello,
Thanks for query on HCM.

Fever with chills,sweating,cough,decreased appetite and constant headache are likely suggestive of upper respiratory infection which could be related to bacterial infection (like streptococcal pharyngitis) or might also be viral infection but it must be investigated further by clinical examination and some relevant blood tests.

So,if I would have your treating doctor,I would like to differentiate the cause through routine blood counts (CBC with differential counts,ESR,liver function tests) and throat examination with pharyngoscopy to rule out infective (especially bacterial) causes.

Therefore it would be advisable to see the doctor (preferably an ENT specialist) who will take complete medical history including history of sensitivity to antibiotics with thoroughly clinical examination of your throat and may order some routine blood works as I mentioned.

So,you will require below treatment.

1) Antibiotics from which quinolones (levofloxacin 500 mg) would be preferred but you will need prescription of the doctor for it and must be started after giving your blood sample (to avoid any false negative results).

2) Analgesics (paracetamol) with anti-inflammatory (ibuprofen-Advil) combination two times daily for your fever,cough and constant headache which is available over the counter.

Advil can be continued but it would be better to avoid Excedrin which contains two NSAIDs (acitoaminophen and aspirin) with caffeine.

3) Take antihistamines like levo-cetrizine 10 mg twice daily for cough.

4) Avoid cold water and other cold products

5) Start warm salt water gargles to reduce throat inflammation.
