Suggest Treatment For Cystic Acne On Chin

Question: Hello, I had clear skin while on birth control for the past 15 years. Three months after discontinuing use of Yaz, I started to develop mild acne. At four months, I started getting cystic acne on my chin for a few weeks, and also began having issues with my periods and ovulation. Around this same time my skin became extremely oily (it has always been previously very dry), and the break outs became less cystic, but more prevalent, spreading to my jawline, forehead, and lower cheeks. The doctor prescribed finacea and clindamyacin phosphate, but I continued to break out after four months of consistent use. I discontinued usage of these products because they didn't seem effective. My skin continues to break out, but now I have noticed tinier little pimples forming next to my old ones that are still fading (I have lots of hyperpigmentation, despite never picking at my skin). I'm trying to conceive, so anything that might lower my testosterone levels (which are elevated due to PCOS) is not an option. Is there anything else I can try?

Thanks for your query.
If your skin is getting with cystic acne after you have stopped using Yaz and treatment with finacea and clindamicine phosphate didnt seem effective , acne are caused from increased level of testosterone(sign of PCOS).
Issues with your periods are causing from PCOS.
In my opinion you should continue to use any other forms of birth control like Dianete 35 because this form of birth control is composed of cyproterone acetate and have antiandrogenic effect(decrease levels of testosterone) and can regulate your periods.
Issues with periods and increased levels of testosterone reduce the possibility of ovulation.
Do not hesitate to do another question.

I'm trying to figure out how to treat my acne. Taking birth control is not an option (read above) because I'm trying to conceive.

Hello!
I understand your concern but many women with high androgen levels don't ovulate, have inhibited endometrial cell growth and secretory activity and don't carry a pregnancy till full-term.

If you don't produce a mature egg and ovulate and don't have endometrial growth, you can't get pregnant.

You should lower your levels of testosterone.
For this reason I advised you medications to lower testosterone, such as certain birth control pills like Dianete.

You can try and with metformine which improves insulin resistance, the cause of many PCOS symptoms, may induce ovulation.

This medication can also help with acne treatment as it regulates your insulin levels, which will help to normalize your hormones and lower your androgen levels.

If you are considering taking a medication, it is important to discuss all the possible side effects with your doctor first.
If you don't conceive after treatment with metformine you may need more potent fertility medications such as Clomid or injectable medications called gonadotropins.

There is no a specific medication for the treatment of acne(from high levels of testosterone) and stimulate ovulation, only with metformine.
