Question: Hi, good evening! I have a friend I am concerned about. Anyone she gets close to, they become her obsession of negativity & mistrust. She honestly believes these delusions. I know these accusations are unfounded and am now quite concerned because we are now middle aged with families & her mistrust and delusions have escalated. I recently became friends with her again & noticed an escalation in fanatical delusions. I stopped being close to her years ago because I was scared of being her object of her delusions. She's lost all personal friends due to this mental illness & this also extends into her professional life. Her husband, whom myself & my husband have known for years is cheating on her, her bosses all want to sleep with her or are bad people. I am now 40, & realize she needs help. How can I approach her about the fact that she has a mental illness without alienating her?

I think, she may have persistent delusional disorder or paranoid personality. In both these case, it is very difficult to convince a patient about her illness. What you can do, you can take help of a psychiatrist and get her properly evaluated. If she is not willing to consult a psychiatrist, then try to take her with you and then discuss with psychiatrist about her.
If she needs medication, in that case, either you can go for oral medication or long acting injectable. With medication, her mental balance would gradually become normal. Rest depends upon her mental condition.
