Question: 23 years old.
Male.

I was sitting eating lunch with mom then suddenly I got this very anxious feeling in my stomach, to where I looked at mom and I felt like she became a stranger to me.. Like I was another person sitting there not emotionally recognizing my own mom.. It was scary. I often feel confused with who I am, whether I'm being myself or just faking it or not.. It all started XXXXXXX of this year when in theiddle of the night I felt this weird sensation as if I slipped in the detached state.. Where I didn't feel like myself anymore.. And having soo many emotionally disturbing detached feelings and racing thoughts that won't go away until I go to sleep for bed. At night when settled down for the night I'd when I get the most relief.. I also have unwanted urges and thoughts of harm.. I hate it. I avoid watching stuff that make me feel like I have bad urges.. I'm not.. I'm not that type of person to think those thoughts.. I hate even bringing that up.. I often try to fake it my best, because don't want to be labeled as a guy who needs medicine or mental help.. My aunt XXXXXXX has a history of mental health but she is healed now and living normal. I am a musician and I have an awesome life.. I just feel soo detached from who I was and how I thought before XXXXXXX 2016 where I began this roller Coaster of unstable moods and scary intrusive thoughts from harm to religious unwanted thoughts.. I hate it.. Plus the feeling of loss of my since of peace where I can enjoy friends and family the wonderful way I did at the beginning of the year I miss it.. Things feeling and unstable thoughts and emotions I wish to go away. What's going on with me.

Detailed Answer:
Hello, and thanks for your question.

The symptoms you describe above are depersonalization, derealization, and obsessive thoughts. These are consistent with a potential diagnosis of obsessive compulsive disorder (OCD), or panic disorder. I do believe you can feel better with treatment. Treatment of this condition in your case would consist of a high dose SSRI antidepressant such as Lexapro uptitrated to 20mg daily. This would need to be combined with exposure-responsive-prevention psychotherapy (ERP). This combination treatment of an SSRI with psychotherapy is highly effective for OCD, and I strongly believe your symptoms would be improved by them.
