Suggest Treatment For Diabetes

Question: Hi,

My father is having diabetes since last 9-10 years. He has traveled from his home town (Bihar) to XXXXXXX for his cataract opaeration. But he has gained in his sugar level recently so he is not able to go under the surgery. I am putting up the chat of his suger level:

As he has high sugar doctors are not able to operate him for the cataract and as per the doctors he should go for the operation ASAP and its getting bad day by day.
Since yesterday my father has got cold and fever too, which he usually gets maybe due to high sugar and it very difficult to get under control after much medication.

- What do you suggest next for my father, shall he take insulin injection for the sugar control?
- Is his medication correct for High Sugar?
- What should he take for the cold and fever?

Detailed Answer:
Hi Mr. XXXXXXX I had gone through your query and understood your concerns.

Sir, Let me first appreciate you for your concern to father's health. After going through the serial blood reports, It is clearly evident that Blood sugar levels are perfectly responding to the increased dosages of drugs.

Fasting sugar level of 115mg/dl is almost acceptable. Post prandial is higher. But if we allow some time gap of 2-3 weeks with this medication, then they correctly adapt to control sugar levels. But if you immediately want him to have cataract surgery and if I were his treating doctor, rather than introducing insulin I would advise him alpha-glucosidase inhibitor like Voglibose 0.3mg. thrice daily just before food which acts effectively on post prandial sugar levels. His treating doctor would know this.

For fever, Paracetomol 500mg 6th hourly for 3-4 days would help. Cold sponging helps. For cold, any anti histamine like ciitrizine hydrocholride 10mg. once daily for a week would help. Avoid chilled fluids. If fever not relieved after 5 days then, he needs evaluation with blood tests for malaria and typhoid.
