Suggest Treatment For Diarrhea In 2 Year Old

Question: Hi, My 2.5 year old kid is having loose motions from last monday evening ( 1 week). Initially it started as vomiting continuously whatever he takes . then from Monday night loose motions also started with foul smell. from wednesday we started giving zofix 2.5ml two times daily, a zinc suppliment 2.5ml in the night and 2.5ml of emeset two times daily. Still he is having loose motions . I am confused whether the dosage of medicines are adequate. What is the correct dosage of zofix for 2.5 year old? Also, when should i give emeset ,after food or before food? is 2.5ml x two times of emeset adequate? He is vomiting nearly after 8 hours of dosage. Regards, XXXXX

Brief Answer: Stop antibiotic , Start Pro-biotic Detailed Answer: Hi Namasthe Welcome to Healthcare-Magic Greetings of the day Dear Mr XXXX Diarrhea in children is usually caused by Rota Virus and is usually self limiting. In some cases there is persistent diarrhea and these are the two commonest causes for the same 1. Prolonged Use of Antibiotic: Prolonged use of antibiotic is not recommended as this may cause antibiotic associated diarrhea. Antibiotics like Zofix(Cefixime) may cause damage to healthy normal bacteria in the gut. These healthy normal bacteria prevent infection and colonization by harmful bacteria, destruction of which may cause invasive by disease causing organism. 2. Lactose intolerance: Many of children during gastro-enteritis develop lactose intolerance. Lactose is milk sugar. If the following criteria are meet, there is no cause for concern 1. No signs of de hydration 2. Baby accepting feeds well 3. Urine output adequate: passes urine 5 to 6 times a day 4. No blood / Mucous in stools 5. No distension of abdomen Do not be worried about the smell of stools, it will normalize I would suggest the following 1. Home made Oral rehydration solution: To a glass of boiled and cooled water add 20 gms or glucose or sucrose(Sugar) and a pinch of common table salt. Feed the child as much as possible. Alternate with Tender Coconut water 2. Maintain Nutrition: Dhal water, wheat porridge, rice water 3. Stop Zofix: Reason previously mentioned 4. There is no need of giving Emset for prolonged time if there is no vomitin/nausea: Ideally Emset is given 30 minutes before food 5. Encourage intake of Curds/Yoghurt 5. Syrup Neutrolin B: Its a probiotic , which restores the normal gut flora/bacteria. Dosage is 5 ml twice a day 6. Avoid Milk: Its better to avod/reduce milk intake for next 4 to 5 days. Lactose present in milk can cause persistent symptoms.( Ability of intestine to digest and absorb lactose in acute gastro-enteritis may be reduced if there are persistent symptoms) Consult Physician when you notice following red signs 1. Dry oral mucosa which implies dehdration 2. Absence of tears while crying 3. Blood/mucous in stools 4. Irritability 5. Decreased movements and excessive sleepiness In case you need any further assistance, will be glad to assist you. Hi, Thank you very much for the answer. I shall proceed as per your suggestion. The vomiting is not completely gone. Today early morning he vomited once. after that I gave him emeset 2.5ml. Vomiting was stopped till 5pm in the evening. after that vomited once. again i have given 2.5 ml emeset. In the night when should i give emeset? as the last thing in the night after all medicines and food? Also, in the morning as the first thing before food? ( Hope i can give him water soon after giving emeset). Regards, XXXXX

Brief Answer: Emset can be gievn at night Detailed Answer: Hi Dear Mr XXXX Give him Emset as the last thing and also 1 st thing in morning. Give him frequent quantity of small feeds. It will help prevent vomiting. I hope he is excepting feeds well, there is no dehydration and he is sleeping well. You can give him water 15 minutes after emset. I have given emeset in the morning and he didnt vomit till 9.30 pm. Vomited once after that. Now i am giving one more dose of emeset. Motion is still very loose..watery.. But he urinated nearly 5-6 times. I am giving him Nutrolin B and Zinc suppliment. Can I give him ORS powder directly,it seems he is ok with powder and not with water. Also, Can I give him glucose powder? It looks like we lost much weight in these past 1 week. Regards, XXXX

Brief Answer: Not advisable to give plain ORS Detailed Answer: Hi Dear Mr XXXXX The oral rehydration solution after mixing with water provides the right quantity of electrolyte and water almost equivalent to that lost in loose stools. Therefore it is not advisable to give only ORS as it will provide high concentration of electrolyte and no water.this will lead to electrolyte imbalance. Instead try home based ORS if commercial ORS is not palatable. Weight loss is expected due to loss of fluids from body.baby will definitely pick it up in the next week.
