Brief Answer:
it is sleep apnea, secondary to your Obstructive sleep apnea condition

Detailed Answer:
thanks for the information.
So it happens that you are suffering form Obstructive sleep apnea(OSA). The difficulty in breathing is probably the apneic attack that tends to awaken you in morning.
As i can notice that you have been put on CPAP, it is perhaps the treatment of choice in this condition and i suggest you continue its use as per you consultants advice.
You also appear to be suffering from multiple co morbid conditions and taking medication for it.
I would need to know
Are you not satisfied with the treatment? Is there no improvement in symptoms profile?
What exactly are your problems now?
Have you done Polysomnography investigation in past.
