This is Dr.Ajay Panwar here, and thanks for replying back with MRI Lumbosacral(L-S) spine.
As I already pointed out that you are probably having a lumbo-sacral radiculopathy ,most likely L5-S1.This probability still gets stronger with the L4-L5 and L5-S1 disc bulges.These bulges may be causing compression to the nerve radicles.
Please upload your MRI CD on Google drive and share the link here,so that I can see that what is the status of nerve radicles and what is the degree of compression these disc bulges are causing?
Treatment-
1)Mckenzie spinal exercises with proper technique
2)Antineuralgic drugs like pregabalin or duloxetine
3)calcium with vitamin D
4)Methylcobalamin supplementation
If no improvement in symptoms occur on regular exercises and medicines,spinal surgery consultation.
Neurological examination is always better if you have access to.Of course,historical opinion is incomplete without neurological examination.However,diagnosis looks kind of settled here.
Fasciculations are a part of radiculopathy and should improve with the treatment mentioned above.Nothing separately to be given for them.
Please follow-up after uploading MRI.

Thanks for being in follow-up.
I don't want to negate your internist's opinion without having seen your MRI.Also,I can't say that he might be correct.You already said in the previous query that there are larger disc protrusions at L4-L5 and L5-S1.So,it has to be seen whether these protrusions are significant enough to cause radicles compression.
With your clinical syndrome,i still stick to the diagnosis of lumbosacral radiculopathy.MRI is to be seen to stick to or change this opinion.
However,there is no harm to get the EMG done.It will further help in the diagnostic evaluation.
