Suggest Treatment For Discomfort In The Abdomen While On Triphala

default
Posted on Fri, 19 Aug 2016
Question: I have been uneasiness in my abdomen since 2 months.I always feel I do not evacuate fully. I had got done my X ray and the doc told me that there is large impaction of faces on two sides of large intestines and he told me to take Amitiza, but it had gelatin and am vegetarian,so I did not take it. He then told to take psyllium husk. I still feel uncomfortable. I haven't abused laxatives with harsh stimulants,I have taken them but not for so long. On frequent basis I used to take Triphala a little more frequent. I am having that uneasiness since 2-3 months and had went for a week without no bowel movement 2 months ago. Is it some thing serious I may be suffering from ? I do not have any bleeding during bowel movement. I recently took a second opinion from a gastro and he told me that it is not possible to know about faecal impaction with a X ray. I do not know whom to believe and what to do. I have tried a lot of things but nothing seems to give sense of complete evacuation.

Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

X rays will show colon loaded with feces. Fecal impaction can be diagnosed with digital rectal examination.

Do the following for relief -

1. Movicol 0-0- 2 sachets ( in 250 ml of water) for one week
followed one week later by
2. Cremadiet Duo 0-0- 2 scoops ( in 250 ml of water)
3. High fiber diet ( Take plenty of fruits, vegetables and greens)
4. Plenty of fluids
5. 30 minutes of brisk walking everyday.

Get blood glucose and thyroid function tests done.

Regards,
Dr.R.K.

Hello,
I live in US, and I cannot see Movicol and Cremodiet duo powder here, although I have tried Miralax,Triphala, Psyllium husk and few other laxatives too but it's been few months I feel I have not had complete evacuation, I always feel there is something in my abdomen. I have shown to multiple doctors here but with no help. Also I do not think it is constipation because I have bowel movements daily but it always feel like it is incomplete. I also eat healthy but that does not seem to help much.

Brief Answer:
Colon Transit Study

Detailed Answer:
Hi,
If you do not find relief with fiber supplements and medications, then the next step is to get a colon transit study is the next step if you feel the evacuation is incomplete.

If abdominal bloating is what you trying to mean, the you will get good relief with prokinetics like Reglan.

Let me know if you have any more questions.
Regards,
Dr.R.K.

Hello Doctor,
Initially I had went to a doctor 2 months ago and he had told that I have gastritis and he had put me on Omeprazole. But my digestive issues continued and I changed the doctor and he had gt done my X ray and came to know that I had faecal impaction but there has been nothing that seem to help to me. Is there any tests or anything else with which I can come to know what is wrong with me. It is not possible for me to live like this.I had got done my H pylori test done but I did not have that infection.I have also uploaded 2 reports which I had got done. Can you tell me which test should I do to know exactly with what is wrong with me.It is very frustrating to live like this.

Need more information

Detailed Answer:
Hi,
In my previous post I mentioned about abdominal bloating.
I was expecting information about whether you have abdominal bloating.

Since I do not have the option to examine you I have to depend on the information provided. I feel the information provided is somewhat vague and incomplete.

By saying- not having complete evacuation/I always feel I do not evacuate fully, do you mean-
1.you have the feeling that there is something still left in the anorectum
or
2. Always have urge to defecate even after bowel movements
or
3. You tummy is heavy/bloated?

Was digital rectal examination/endoscopy/colonoscopy done?

Regards,
Dr.R.K.

Hello,
Yes I do have bloating sometimes, but daily when I have bowel movement, I feel it is incomplete. The faeces is of different color sometimes during the same bowel movement and different size on different days. Also my appetite has reduced to a large extent. I have tried 3-4 doses of Miralax too but that feeling of incomplete evacuation is always there no matter what. No I have not got done any test like colonoscopy, endoscopy or rectal examination. Also most of the times I am full. Also yeah the urge is there like it do feel like the urge to go but nothing happens.

Hi,
Thanks for the additional information.

Based on the information you have provided, it looks like you have functional dyspepsia and constipation predominant IBS.

You need to undergo a few tests like upper GI endoscopy, colon transit study, blood glucose and thyroid function test.

You need to take the following medications -
1. Nexium 1-0-0 before breakfast
2. C.VSL #3 1-0-0
3. Cremadiet Duo 0-0- 2 scoops ( in 250 ml of water)

Your name sounds XXXXXXX though you stay in the US. You can get these drugs from XXXXXXX

Amitiza and Linzess are good medications of IBS-C. But they are in capsule forms and you said you wont take it as it is made of gelatin.

Dr.R.K.
