Brief Answer:
Get an endoscopy and ECG

Detailed Answer:
Hello, Sir.

I can certainly understand your concerns.

I apologize for the delayed response owing to my hectic work schedule.

Your symptoms could possibly reflect acid reflux disease considering your age and associated smoking history.

But nevertheless it is important to rule out cardiac disease at all cost.

For confirmation I advise you to get a baseline ECG and upper GIT endoscopy for further assistance.

Further management if any is based on test results.

Post your further queries if any.
Thanking you
