You have two problems the first is ear pain. The ear pain has persisted despite all your efforts. My thought here is that you may not have problems with your ear at all. Since the TMJ makes up the floor of the ear canal it is possible that your ear pain is actually TMJ pain. This could be arthritis of the TMJ joint. If you grind your teeth at night this is further evidence that you may have TMJ issues. I would recommend seeing your dentist and get fitted for a bite guard. This may reduce the nightly grinding and help reduce your pain
Your second problem is tinnitus. Unfortunately here there is nothing you can do to lessen the symptoms. Tinnitus is caused from permanent ear damage. The best advice I can give you is at night use a radio or other white noise for your brain to focus on so you drown out the tinnitus.
