Suggest Treatment For Elevated TSH Levels While Trying To Conceive

Question: 36 yrs 8th month running. I have recently got married . tsh checked and found 19.2 where average shud be 4 to 9 i guess . my t3 t4 are normal. I want to conceive asap . also i am on tegritol 200 mg per day. Please suggest how to work it out i am trying forpregenancy for over 6 months now. doc has suggesed 50mg thyronorm now.

Thanks for the query.
I have read your query and understand your concern.
Hypothyroidism is associated with infertility and this is what is happening with you.
It is better to have your thyroid status well controlled before conceiving because if hypothyroidism persists during pregnancy,you would have more chances of miscarriage and other pregnancy complications like prececlampsia. Even a child born to a hypothyroid mother ( uncontrolled TSH ) may have low IQ.
The best for you would be to get your thyroid status well in control before conceiving.
The ideal range for TSH ( while conceiving ) should be between 1-2.
You have been rightly prescribed THYRONORM 50 now but this dose needs to be titrated and adjusted.Full effect of THYRONORM is achieved in 6 weeks and hence you should check your TSH after 6 weeks.
If still not controlled,then the dose of THYRONORM has to be increased till your TSH becomes stabilised between 1-2.
Once you achieve this target ,you can plan your conceivement.
Remember that ,once pregnancy occurs,you require more THYRONORM for proper development of your child.
It would be better for you if you are jointly managed by your doctor ( obstetrician ) and an endocrinologist for the proper management of your thyroid status.
