Detailed Answer:
Hello!

Thank you for asking on HCM!

Regarding your concern, I would explain that pain and inflammation can lead to high blood pressure.

Your actual blood pressure values are above the normal ranges and you may need to start antihypertensive therapy (betablockers have a low effect in controlling blood pressure, their main effect is on the heart rate).

I would recommend consulting with your attending physician for a careful physical exam and some tests to investigate for secondary causes of high blood pressure:

- a chest X ray study
- a resting ECG
- some blood lab tests (complete blood count, PCR, sedimentation rate for inflammation, thyroid hormone levels for possible thyroid dysfunction, fasting glucose, blood electrolytes, kidney and liver function tests).

If all the above tests result normal, you should discuss with your doctor on the possibility of starting anti-hypertensive therapy (I would recommend an ACEI or ARB, or a calcium channel blocker).

A better control of the pain, would help avoid high blood pressure spikes.

Tylenol is not related to high blood pressure.

I would also advise some tips to help maintain a normal blood pressure:

- reduce salt and caffeine intake
- avoid alcohol or smoking contacts
- try to lose some pounds if you are overweight
- perform a lot of physical activity (you can try swimming, aerobics, etc.).

Hope to have been helpful!

Best wishes,

Dr. Iliri

Thank you very much. My pulse stays in the 50 to 55 Range with Beta Blockers. Is that safe? This week I am starting in a Program that is doing clinical research for Pain management for products that control the pain for osteoarthritis. I will be given many procedures that you mention. MRI, XRAYS ETC. I also take Amlodipine Benazepril. When the hip pain starts so does the spike in BP like you mentioned.
I have always been under the impression that the side effect pain was a drug reaction as Carvedilol alone has almost 50 side effects.I only use Tylenol for pain only. Only thing that seems to work on Arthritis. I do not smoke, drink or have coffee. I go to the Gym 4 days a week and lost 20 pounds in 2 months so everything is normal there. How do I start antihypertensive therapy? What is it? You are a cardiologist and can you give me any kind of direction until I stop the pain and my Blood pressure goes back to normal. I want to make sure the low pulse do not hurt my system. Thank you.

Hello again!

Thank you for the additional information.

In fact you are actually taking antihypertensive therapy (drugs against blood pressure), which are amlodipine and benazepril.

Coming to this point, I would recommend increasing the dose of amlodipine and benazepril. If this is not possible, adding a new antihypertensive drug may be necessary. I would recommend hydrochlorothyazide in this case.

Regarding your heart rate, it is not concerning. Do you have chronic fatigue, lightheadedness, dizziness (symptoms that can be explained by bradycardia)?

In such case a reduce in the dose of carvedilol may be needed.

You should discuss with your doctor on the above issues.

Hope you will find this answer helpful!

Kind regards,

Dr. Iliri
