Your creatinine value and potassium value is a bit more than normal value. With diabetes in the background, I will not take this lightly. I will suggest you consider a much strict diabetic diet. a low protein consumption as well as low salt intake for now.
You should repeat the test after couple of days. if the value persists / worsens, you should consider consulting a nephrologist who can advice more specific treatment like water / fluid restriction, strict calorie as well as protein restriction.
