
Suggest Treatment For Enlarged Multifocal Fibroid In The Uterus

Question: Hello,
I was recently diagnosed with enlarged multifocal fibroid uterus, this is what was found during the transvaginal ultrasound: My uterus measures 19.3 cm. in length, 16.3 cm. in width, with AP diameter 11.8 cm. My estimated uterine volume is 1942 ml. The largest fibroid arises from the posterior upper uterine body and fundus, measuring 10-13 cm. Smaller fibroids include: 6.4-8.0 cm. intramural, partially submucosal fibroid located on the left lower anterior uterine body. 5.0-5.5 cm. left intramural, subserosal fibroid located in the midline of the anterior uterine body. The fibroids distort the endometrial stripe, maximum 1.4 cm., portions of the endometrium compressed by abutting fibroids. I'm experiencing all the symptoms-- heavy menstrual bleeding, prolonged menstrual periods (seven days or more of bleeding), pelvic pressure and pain, frequent urination, constipation etc. I found out last week that I'm severely anemic because of the heavy/prolonged periods and clotting and had to get a blood transfusion (my hemoglobin was 5.1), so let's just say these suckers are definitely affecting my everyday life!
I am 28 years old and recently married. My husband and I would love to start a family of our own soon, so my question is: Based on the size/location of my fibroids, what options do I have for removal of the fibroids, and what would be your suggestion based on my condition?

An experienced surgeon might attempt myomectomy, that is, only removal of the fibroids.

I currently reside in Oklahoma City, Oklahoma. Fertility is definitely desired! I know that a myomectomy would only be a temporary fix and that I'll more than likely need a hysterectomy at some point, I was just hoping that removal would be a possibility so that we could try to get pregnant before they came back. Do you think I will have a difficult time finding a surgeon that will do a myomectomy? And if so, is it due to the size of my fibroids or their location, or both?
