Suggest Treatment For Enlarged Prostate And PSA Level Of 4.1

Question: Sir, I am old patient of Enlarged Prostate and am taking Dutas T for last several years. I had uroflow about a yr back 13.1 and now it is 11.2. I am advised by some of my doctor friend to take Silodal D 8mg.
Should I take it sir, and will it improve the flow and obstruction. My PSA is now 4.1.and prostate size is 168. Also please let me know whether I should go for Surgery by Bipolar TURP or by Hellop and which is more safe and accurate. My age is 68 yrs and am taking Volix0.3 and Gilmy for sugar which is 120 fasting. I have been also taking Preva 75 for thinning blood. For BP also I take one tab.and it is in normal regularly.
Sir, does the size of penis (deflated about 3") has any effect on the surgery
I shall be highly obliged if you please guide me in the matter

Thanks for your query ,based on the facts that you have posted it appears that you have obstructive urinary symptoms due to enlarged prostate .

To summarize you have following problems
1) Diabetes and hypertension
2) High cholesterol
3) Grossly enlarged prostate with volume of 168 gms with near normal urine flow (May be because of drugs Dutas T.) No mention of post void residual urine volume

Enlarged prostate is a progressive disease that involves most of the men after the age of 60 years

Taking medications for enlarged prostate does not arrest the progress of the disease , more over after few years these medicines are not effective to improve symptoms

In view of your stable health parameters now, I would suggest you to get surgery of endoscopic resection of prostate done to make the passage clear and obstruction free which will help your life in future coming years to be free from obstructing urinary symptoms and to good quality life .

As regards your query about bipolar or laser surgery I would state that the instruments ,principles and technique used in both the methods are same ,the only difference is the source of energy used to resect the Prostate gland .

If you have no financial issue you may opt for laser surgery which is rather expensive but with less morbidity compared to conventional electrical energy ..

Do not worry about the size of your penis as far as surgery is concerned it becomes large after giving spinal anesthesia..

Sir, thanks for your advise. You did not advise whether I should take SILODAL D 8mg in place of DUTAS T. Will it be more helpful vis a vis Dutas and any side effect?

Silodal D and Duts T are the drugs that belong to same group of drugs and have similar mode of action .There is no much difference as far as results and side effects are concerned .
