Question: I was found to have Mylofibroses 3 yeas ago.
I had Polysyrhemnia Vera 5 yrs before. I have been taking Jakafi for 3 yrs, the first two yrs I had blood
screwy try, the last year red blood count
holding at 9 to 10! My symptoms are fatiqe,
Enlarged spleen, night sweats and usual
Symptoms! What stage am I considered to
be in! I am 68 yrs old! My life expectancy?

I have gone through your question. I can understand your concern.

You have history of polycythemia vera.

There are three main phases.
First is pre polycythemic phase.
second is overt polycythemic phase and
third is post polycythemic myelofibrosis phase.

In post polycythemic myelofibrosis phase patients develop myelofibrosis of bone marrow.

Hemoglobin and other counts are reduced and there is extramedullary hematopoiesis which leads to enlarged liver and spleen.

So you are in third phase.

Life expectancy depens on treatment and overall general health of patients.

Life expectancy can be from 1 year to 15 year.

Few patients may develop acute leukemia and it shortens the life expectancy.

That would be Marrow!!!!!!

Your marrow is replacing with fibrosis.

Hi, dear
I have gone through your question. I can understand your concern.

Its not like stage 1, 2, 3, 4 like in other cancer.

It is post polycythemic myelofibrosis phase.

In this phase gradually your marrow will be replaced by fibrosis.

So blood production in your bone marrow is decreasing. It occurs from other site like spleen and liver. Which leads to enlargement of them.

So your counts will decrease further as amount of fibrosis will increas.

However it is chronic disease and not spread faster like other leukemia. It is slow process and that why you may remain healthy for years.
