Question: I am a 56 year old male with Diabetes. I have ED. In the last couple of years I note that when I do get an erection it is very thick at the base (thicker than when I used to get an erection), tapers up to "normal about 2/3rds way up and the top 1/3 is firm but not hard. Is this normal?

Hello,

Thanks for writing to us.

You have Diabetes with ED and taking viagra. If you do not have any pain while erection and no difficulty in urinating probably no significant problem is there. It is merely your observation and it is normal.

However considering history of diabetes there is possibility of some inflammatory changes around urethra.

You need ultrasound and penile Doppler to check for problem if any.
At this age it is also necessary to rule out prostate problem by PSA blood investigation.

I request you to upload clear photographs of erect and flaccid penis so I can give more comments.

Thank you. No pain during urinating. Some discomfort underside near base of penis during erection. My Dr does regular PSA checks when doing other blood work for the Diabetes. Will try and upload photos.

Thanks for follow up and images.

Actually there is relatively shrinkage of mid shaft of penis due to diabetes and aging.Diabetes causes stiffening of blood vessels. During erection blood cannot flow easily in to penis shaft as vessels do not have sufficient elasticity for dilatation and blood accumulated near base of penis which causes some discomfort during erection.

One medicine cum vitamin supplement containing pyridoxal-5-phosphate, mecobalamin and l-methylfolate (trade name tetrafol plus) may help you.. It has shown improvement in 50-60% patients of mine with ED having hypertension and diabetes.You should take it after meal twice a day. If you feel some improvement in 15 days you should continue for at least 2 months. You may get this medicine over the counter as it is vitamin product.
