Question: Dear Dr,

I am 51 years of age, married with two children. Since last two years, even though I have the urge to have sex, I am not getting an erection. Please advise a good remedy for the same. I have tried taking all types of supplements including Viagara but to no avail. Please advise.

Brief Answer:
Intracavernosal therapy will help.

Detailed Answer:
Hi Sir,

I understand your concern.

The cause of impaired sexual drive/erection in your case can be multi factorial.

Most cases are associated with use of antihypertensive drugs or improper control of blood sugar or blood pressure.

I would like to know if you have any such associated medical co morbidities.

A better control of such aliments will often improve sexual drive.

Coming to your issue specifically I would like to recommend Intracavenosal Injection therapy.

Intracavernosal injection therapy is an effective therapy for men with erectile dysfunction (ED) who can not take oral agents or for whom oral agents are not effective.

For this I would recommend Inj Alprostadil/PG E1 available as Caverject Impulse.

Injections are given with a 1 ml syringe and a 27 to 30-gauge needle.

The injection may be given anywhere from the base of the penis to two-thirds of the way down the penile shaft at the 10 o'clock and 2 o'clock locations on the upper side of the penis away from the urethra and the head of the penis.

Injections are rotated within that area and the side of the injection is alternated with each injection.

Initial dosage recommended for Caverject Impulse is 2.5 mcg with a second dose of 2.5 mcg to be given if the initial dose is inadequate.

It is than recommended that the dose be increased by 5 to 10 mcg intervals at each attempt (a minimum of 24 hours apart) depending on the erectile response with each dose (up to a maximum dose of 40 mcg).

The response is adequate and prompt with this therapy.

Please note that you need to have a normal penile Doppler study prior to this therapy for effective and adequate response.

Consultation with an expert XXXXXXX will help.

Post your further queries if any.
Thank you.
