Suggest Treatment For Extreme Pain In Upper Right Abdominal Area

Posted on Mon, 28 Sep 2015
Question: I have been suffering from extreme pain in my right upper abdominal area going around to my back. Eating does seem to start the process. I had my gall bladder removed 15 years ago, but the symptoms are very similar to those before my surgery. My doctor requested a ct scan which did not show anything. This was yesterday and I still am suffering. Do you have any ideas?

Brief Answer:
Upper GI endoscopy, blood tests

Detailed Answer:
Hi.
Thanks for your query.

To recapitulate: 68/Female - pain in upper right abdomen similar to gall bladder problem since yesterday in spite the gall bladder removed 15 years ago; pain going to back, eating starts the pain - CT scan normal -

I would like to know a few more details for me to assist you more:
- Does the pain start or increase immediately on good or after some time, if after some time then how much?
- Any Black stools (malena)
- What is the type of the pain - like burning, twisting, throbbing or so?
- Any nausea/vomiting?

My thoughts:
The possibilities are
-Gastric or duodenal inflammation or ulcer, particularly on the posterior wall.
-Pancreatitis
-Rarely Sphincter of Oddi dysfunction or so just to mention.

I would advise in such a case the following:
- Blood tests - particularly the liver function tests and amylase and lipase
- Upper GI Endoscopy

Start PPI like Omeprazole, antacid get and Domperidone.
Soft bland diet
Control of stress and anxiety if you have.

I hope this answer helps you to get a proper diagnosis, and get a proper further treatment. Please feel free to ask for more or if you need further clarifications or if you feel there is a gap of communication.

Brief Answer:
Thanks for your feedback.

Detailed Answer:
Thanks for your feedback.
These reports look to be normal by standard range, please check with the normal range given in the report.
Urine infection can not cause the type of the pain you have explained. But it is good that antibiotic has been prescribed.
