At age of 61 with hysterectomy and liver transplant there can be degenerative wear of bones, muscles, ligament and facet joint arthritis, due to deficiency of calcium, vitamin D, VIT B12.so supplements of these nutrients can relieve pain.
Osteoporosis may be associated leading to diffuse pain in back.
Check your bone strength with DEXA SCAN and you can start medication according to level of osteoporosis,
And also check your vit B12 and vit D3 level.
You can reduce flare up by doing regular exercises and medication.
You need to start supplement of calcium with vitamin D, vit B12.
I will advise you to do regular physiotherapy and exercises as my patients find much relief by these therapies.

I apologize if my previous answers were not helpful. I understand the pain and suffering. In this context, I shall list the following options which you can discuss with your doctors.
1. I understand epidural shots are not feasible with low platelets. But this option can be tried after correcting low platelets. Single donor platelet transfusion where blood (platelet) cells can be transfused to improve the count and the shots will become an option as the counts cross 100 thousand cells/cumm. I am not sure if you received platelet transfusions earlier. If you haven't discuss with your hepatologist.
2. Since the benefits of physiotherapy are very significant, you can learn a few techniques after a couple of sessions. Off course, you might need to travel for few days; but I feel that would be worthwhile. You may also consider some physio-equipments (if they are cost effective) which you can use after training at home.
3. Besides these, yoga pranayama and XXXXXXX XXXXXXX are few wholistic techniques that have been helpful in many of my patients.
These are 3 options, I would try along with calcium and vitamin D supplements. You can discuss about those with your orthopedic and hepatologist doctors.
