I had gone through your question and understand your concerns.

You have long history of wine drinking with high bilirubin with fatty liver on Ultra sound.
You should stop wine drinking rather than decreasing for your long and healthy life.
Because this wine-
1) This wine cause obesity and obesity carries its own risks, including heart disease and some cancers.
2) Disrupt your 'sex hormones', with the possibility of loss of fertility.
3) Cause chronic liver damage.
4) Chronic gastritis as already you are also suffering from burp.
If you stop wine then your fatty liver will recover almost fully.
Now comes on Nicorette-
You should try to decreasing this and finally stop.

I feel that this discomfort is due to acidity and fatty liver. Acidity too can produce burps.
Avoid tea, coffee, oily and spicy food.
This will decrease discomfort due to acidity and also helpful in recovery from fatty liver.
Eat plenty of salads in diet. Consume plenty of water(2-3 lts/day).
I also advise an antacid to my patients like Tablet- Pan D- (Pantoprazole 40 mg + Domperidone 10 mg) twice a day empty stomach for 5 to 7 day.
Along with these follow routine physical exercises for 30 to 40 mins.
You will be fine soon.

Constant chewing of Nicotine gum ( it contains nicotine) is also cause of acidity and make symptoms worse in patient already suffering from gastritis and this gastritis is a common cause of burp. .
