Suggest Treatment For Fever And Right-sided Chest Pain

Question: I had fever and pain in chest on right side from past 10 years. I consulted many doctors and finally a doctor recommended me to do MRI. Consulted a general physician with MRI report. She suggested me to consult a cardiologist.

My name is Vinay and I believe I spoke to you over the phone This afternoon around 11'Oclock.

I believe the MRI report and query you have created are with regard to your Domestic Staff.

To reiterate the converstation. I believe that the MRI does not show anything significant. Just some enlargement of the lower chambers of the heart (the Ventricles). This is to be expected in a person with a history of Hypertension as you told me she has.

I would strongly suggest you upload all her records to this query. Lets get everything together and examine what is going on. In the meantime. I have a few questions for her and I would be grateful if you could act as intermediary and write the answers back to me.

1) Has she actually had fever and chest pain for 10 YEARS? What has been done over the past 10 years to treat this fever? What medications has she been on?

I need a detailed history, because quite frankly... having a fever for 10 years will kill someone and i'm not sure she knows what she means by fever.

2) Describe her day to day symptoms. You mentioned fatigue and chest pain. Tell me about the, When are they worst.. When are they relieved?

3) Tell me about her lifestyle? What are her food habits, work and sleep habits? Does she have any children? Do these children have similar complaints?

4) Does anyone else in her houshold have similar complaints (fevers are generally caused by infections and if she has had a fever for such a long time.. i would expect that infection to have been transmitted to her family at some point).

5) You mentioned that another doctor said she had an infection in her heart. My guess is that he meant endocarditis. Can you tell me that story in detail? Also, i need her latest ECHO uploaded.

6) I will try and stitch all this data together as it comes in. If i feel that a bonafide cardiologist needs to get involved. I will turn the converstation over to them.

Unfortunately, something seems amiss. I reviewed the following 3 images.

All three represent the same image. An MRI of the Thorax done on 03/09/2015. I remember that during our phone conversation, you had mentioned some other testing, like an ECHO etc. Could you send those?
