What Causes Low Grade Fever, Intermittent Night Sweats, Swollen Legs And Anemia?

Question: yes please,A 65-year-old XXXXXXX man living in Australia. He has been having low-grade fever with night sweats on and off, oedema of the legs, slight anaemia. Only positive findings:: elivated CRP (165)and ESR (35) and slight neutropenia, – the symptoms have been there for the last 10 weeks. All tests including bone marrow biopsy, blood culture, EBV panel, serology and blood film for malaria and filariasis, urine for AFB – all negative. Pet scan revealed two small lymph nodes bilaterally along the internal and external iliac blood vessels – radiological opinion was inconclusive the lymph not considered to be too small for aspiration biopsy, open core biopsy is being considered.

Comorbidities: bilateral inguinal hernia, huge rt aided hydrocele, long-standing ED and testicular dysfunction and extensive chronic Psoriasis. Dermatological, cardiac, neurological, rheumatological infectious diseases opinions are negative. Any advice would be appreciated email: YYYY@YYYY

Hello dear. I have gone through the details. So its very difficult to diagnose as most of the results of investigation donot lead to anywhere. Only a few conditions needs to be ruled out
Endocarditis by echo
Infected hydrocele by aspiration of hydrocele
Tuberculosis of hydrocele n spermatic cord
Prostatitis
Psoriasis itself can cause fever.
