Question: hello doctor .my 1 year son 14kg weight is getting fever around 99.8 since last nite .i am giving him 4ml crocin DS( as crocin deips are not avialable) every 4 hrs. he has very little cold. he got rashes in anus area as i feel he ia trying to pass motion many times nut could not pass all time . one time in a day he passes motion perfectly. other time he just looks like trying to push. please advise if crocin DS evey 4 hrs is ok. any other medicine

Detailed Answer:
Hello Mr. XXXXXXX

Most of the time fever in this age is due to some viral infection. Give paracetamol (crocin ds) initially only when temperature is above 99 F. Record the temperature 4-6 hourly. Repeat the paracetamol only when temperature(99 F) is there,otherwise it will be difficult to recognize the pattern of fever.

If fever recurs before 6 hours then you can give it 4 hourly. The dose you are giving is appropriate. Also do tepid sponging(sponging with lukewarm water) to bring down the temperature.

If fever persists even after taking these measures you can give meftal-p(mefenemic acid) @ 10 mg/kg/dose 8 hourly to control the fever.

If fever persists after 72 hours consult your pediatrician for evaluation and to rule out any bacterial infection.
Give him plenty of fluids. It will help to prevent dehydration and also in bowel abnormality.
If he is passing stool perfectly(not very hard) then chances of constipation is not there. Apply Happynap/Rashfree ointment locally around the anus three times a day and keep the area dry by gentle wiping.
If stool is hard then you can give lactulose syrup after consulting your pediatrician.
