I understand your concern.
It seems your father has Hypertensive or Diabetic Nephropathy as his Serum Creatinine is high.
What happens many times is that, due to reduced excretion of the medicines, they accumulate and cause low blood pressure.
Another possibility is reaction to any of the drugs he is taking.
Best way out in such situations is to stop all the medicines, and re introduce each Medicine weekly and to notice that which Medicine is causing this Allergic reaction.
I use Amlodipine for blood pressure in such scenarios.
For blood sugar control use of Insulin is advisable adjusted according to daily blood sugar tested at home with Glucometer.
I will not advise to give him any oral medication for diabetes.
Also get his Ultrasound Abdomen to see for status of kidneys and get a Chest Xray done.
I am hopeful, slow and progressive introduction of medicines will help to control such life threatening incidents and will also help to identify the offending drug.
You should see an Internal Medicine Specialist (MD Medicine).

I have seen high blood sugar report.

Dear Friend
I have seen in attached reports that his blood sugar is high. I have advised insulin because of that considering that he might be getting any medicine for his blood sugars.
You can get a repeat:
1. Fasting and 2 hour Post postprandial blood sugar.
2. HbA1c.
If these reports are normal, he will not require insulin.
Review with the reports.
