Hi i have been with my girlfrind for a year and yhis monday i woke up with saw red penis which burns when i pee it also is quite warm the yellow doscharge smells a bit like yeast and is quite thick and i have had in befoe and i had some cream but the cream is not working this time ans and sometimes my foreskin is hard to pull back

It may be due to urethritis or fungal( yeast) infection.
It is more likely to be fungal infection, If the partner also has/had yeast or fungal infection.
You can clean the area with warm water twice daily .Wear loose cotton inner wears.You may need topical antifungal cream like clotrimazole and oral antifungal like flucanazole if fungal infection . Partner also need to be treated simultaneously. Avoid Intercourse.
If retraction of foreskin is a problem(it may be going for phimosis), you may need some procedure like circumcision .
You may consult your doctor for examination and management.
Both of you can get tested for STDs too.
