Question: Hi,
55 year old male. I have what appears to be a half circle shaped series of itchy lumps on top of my foot that will not go away. Tried hydracortizone creame, allegra cream, etc. for 2-3 months now. Any Idea what this could be or if there is another tx I might try?
Thanks

Brief Answer:
Looks like ring worm that has been scratched.

Detailed Answer:
Hello XXXXXXX

The rash looks like a fungal skin infection called ring worm. Ring worm makes irregularly shaped circles that move somewhat or change shape somewhat over time. The borders of ring worm that make the irregular ring are usually redder than the rest of the rash. So that part is consistent with what you have.

The angry red bumps however, are not generally a part of it, but could be from scratching the ring worm, which is itchy.

It is hard to say definitively without seeing it in person, but that is my guess from your description plus the photo.

I could recommend an antifungal over the counter cream, but at this point, given how angry it is in places, I would recommend you see your doctor for a prescription strength antifungal cream such as ketoconazole.

I hope this information helps. Please let me know if I can provide further information or clarification.

Thanks, Dr. Can you recommend an over-the-counter cream that would work for this?

Brief Answer:
Some information on over the counter treatment:

Detailed Answer:
You can try:

Lamisil (terbinafine). This comes in cream or gel - you can use either.

It may take around 2 weeks to show improvement, and around 4 weeks to eliminate the infection.

If that doesn't help, an oral anti fungal may do the trick better.
