
Suggest Treatment For Gastric Issues In A Diabetic Patient

In April I took her to a private doctor because she lost weight and appetite,the following prescription Histak 75 mg Ranitidine, Resmed Mist Mag Trisilicate BP, Nucotrini, Flagyl 400mg, Amoximill 500mg, Mycorist Fluconazole 150mg was given to her.
On 18 May 2015 at the clinic in Diepkloof Johannesburg Metformin, Pharmapress 10mg,Vitamin B Complex and Ridaq 12,5mg was prescribed. She was constipated not eating solid food she had soft porridge, juice, soup and hand full of paw paw at night she will feel hot, dizzy and vomit yellow bile.
On the third day I took her to the same doctor on 28 May 2015 he prescribed Lacson Syrup,Clopamon Syrup and Enervite Chewable. The doctor reffered us to the public hospital at GIT.
On 01-06-2015 they prescribed Napamol and Hyospasmol blood test was taken, the results will be issued on the 06-07-2015, she is booked for G-Scope on that day. Her appetite does not improve she is still having soft food.

Based on your query, my opinion is as follows:
1. Going by the recent history, she has in recent days been under a stressful phase and a long medication list.
2. All this adds to a possible gastritis with possibility of gastric ulcer (need confirmation by endoscopy)
3. She needs to continue with diabetes medication. Along with it, for possible gastritis, she will need to start proton pump inhibitors like pantoprazole or omeprazole twice daily. She needs to continue till the endoscopy procedure. Based on procedure reports, further management can be thought of.
Due to stress and gastritis, her appetite is low. Let her be more on a liquid diet. If its severely painful, antacids can be taken. Otherwise proton pump inhibitors will help.
Once gastritis comes into control, she will slowly start semisolid and solid foods. Avoid spicy or sour diet. Stress reduction and relaxation needs to be tried. Good hydration (atleast 5-6 liters of water/day) is essential. Blood pressure, a bit more is desirable. Improve with hydration.
Its over the counter medication and can be started immediately.
