Suggest Treatment For Gestational Diabetes

Question: I am 26 weeks pregnant and I had failed the 1 HR glucose screening with a score of 170. I recently took the 3 hr glucose tolerance test the other day and my results were as follows. I need help determining if I have gestational diabetes. although I looked up the normal ranges online and I seem to have fallen beneath the cut off. I just need some clarification on this please. It was also after drinking the 100mg of glucola. also, all 4 of the urine samples taken per hour were negative for glucose. Thank you so much for your assistance!

0 hr (fasting glucose) : 74
1 hr : 153
2 hr: 141
3hr: 63

Detailed Answer:
There are several ways of screening for Gestational diabetes so I can understand the confusion amongst patients and providers.

The 100 mg glucola test you are referring is typically not performed by itself. It is preceded by a 1 hour non fasting and 50 gram glucola test. If the lady exceeds the blood glucose threshold at the 1 hour mark of 140 mg/dl, only then does one proceed to perform the 100 gram and fasting glucola test.
There are 2 cut-off systems used for interpreting this second step ie the 100 gram fasting OGTT (Oral Glucose Tolerance Test) :

0 hr (fasting glucose) : 95
1 hr : 180
2 hr: 155
3hr: 140

And the second set of values used by other researchers and practitioners is:

0 hr (fasting glucose) : 105
1 hr : 190
2 hr: 165
3hr: 145

So although you screened positive for the 1 hour test, you passed the 100 gram OGTT regardless of which cut off system you use. So by this token you do not have GDM.

I personally prefer the one-step 75 gram fasting OGTT with the following cut-off values:

0 hr (fasting glucose) : 92
1 hr : 180
2 hr: 153
