Suggest Treatment For Globes Sensation On Tonsils

Question: Hey doc I been to 2 different ENT doctors to get this checked they say it's my tonsil but I don't think so it drives me crazy I can constantly feel it there its been there for a year now. Is this cancer I have no pain no bleeding no other symptoms but a mass of tissue there with globes sensation. Very dry. I attached he'd a picture please help

Hi

Thanks for posting the query. I have gone through the history, understood your problem and also, examined the photo thoroughly. Do not worry! I will try my best to help you out with this problem.

1. On the left side of the image, I can see the left tonsil with a deep crypt dividing the tonsil into two different halves. Just adjacent to the lower part of the tonsil is the base of the tongue with enlarged lymphoid follicles, also known as lingual tonsils. On the right side of the image, the uvula seems to be edematous and slightly elongated.

2. To understand the normal anatomy of the tonsil; tonsil is a lymphoid organ with many crypts on the free end. The biggest crypt is called 'crypta magna'. These crypts vary in size and shape. In your case the crypta magna has become very wide, so as to, divide the left tonsil into two different lobes - this is also known as tonsillar cleft. Your cause of the concern is the lower lobe of the left tonsil, which is perfectly normal.

3. The sensation of the globe and dryness in the throat could be either due to dehydration (because of inadequate water consumption) or Laryngo-pharyngeal reflux (secondary to the acid reflux from the stomach into the throat). I do strongly feel you could be having Laryngo-pharyngeal reflux, because of the stress you are undergoing. Correction of this condition can solve your problem.

4. I doubt cancer in your condition; the following are the reasons to substantiate the same:
a. The age - you are very young to get either cancer of the tonsil or the tongue.
b. You are suffering from this problem since past one year. If it were to be cancer your oral cavity would not look so healthy and the most important, you would not have been alive. The cancers of the oral cavity are very aggressive and spread very fast, within months, it can turn out become really disastrous.

5. Hence, I would like to stress here again, you are perfectly normal and the cause of all your concern is the Laryngo-pharyngeal reflux.

6. I would suggest you to drink plenty of warm water, try taking proton pump inhibitors such as Pantoperazole or Esomeprazole along with a prokinetic. You will definitely feel better. Avoid coffee/tea/spicy food/chilled food & beverages/smoking/alcohol (if any).

Doc Here is one more picture. U see the white patch under the tonsil when my tongue is back that's what it looks like what I stick my tongue out the white patch forms into that mass of tissue. Is this the tonsil. The feeling is dry and irritating is right were that mass of tissue is. I've tried everything

I sincerely apologize for the delay in replying.

I have seen the new photograph after zooming in and I adhere to the remarks I made in my previous response. It is a part of the left tonsil and it looks like an extra growth because of the presence of a deep crypt.

Secondly, I don't see any abnormal changes in the left tonsil. To be XXXXXXX with you, I have been practicing since past 13 years and routinely examine the throat, nose and ears of all the patients who come for my consultation. Hence, with confidence and experience I can tell you, it is perfectly normal.

Finally, your concern of dryness in that particular area could be because of the Laryngopharyngeal reflux. If you are really concerned about this growth, you can always request your ENT specialist to excise the tonsil and send it for testing.
