Suggest Treatment For Harm OCD

Question: I am a 38 year old woman who has Harm O C D i have thoughts from killing my family friend nieghbours raping kills setting coworkers on fire calling the police shooting etc i am on 60mg of latuda and been on it for 1 month no relief how long does this work why cant i have relief i have pictures all day long of hurting and harming people animals i even see animals eating me i need help some relief from this iam with a doctor who prescribe this but no one understands it does not work i have been fighting with this for several years it all starting after the birth of my first child

I read your query and feel sorry for the problems faced by you due to OCD.
As I can see you have tried all approved medications but there is no mentions of behavioral therapy which is equally important.

I go for my OCD patients in following way
I start with SSRI (I get best with fluoxetine or fluvoxamine) and build up maximum dose in a period of one month.
If there is incomplete or no response I add Anafranil and increase it slowly up to 200 mg per day.

Most of the patients respond at this stage. In case there is incomplete response I add risperidone 1 mg and increase it upto 2mg after a period of two weeks.

I give Exposure and response prevention from the day one and most of my patients respond at one or another stage of treatment.

I request you to discuss this course of treatment with your doctor. I am sure that you will get results sooner or in next three months.
