Suggest Treatment For Heartburn And Pain In Abdomen
I have been experiencing severe heartburn and pain in upper abdoment over the past 1 month. The pain severity ranges from mild to severe. It happened twice in the last one week post which I decided to consult a local gatroenteralogist. An ultrasound scan was recommended for checking of gallstones and it turned out to be positive. During follow-up with doctor, laparoscopic gall bladder removal surgery was advised.
Attached the OP note and ultrasound reports.
Please advise if surgery is mandatory. If it can be cured with medications. Is the operation urgent?
Presently surgery is not mandatory
Detailed Answer:
Hello. Thanks for putting your query in HCM.
I can understand your concerns.
I have gone through the reports you have uploaded
Ultrasound abdomen do suggest multiple stones in gallbladder however the wall thickness is normaal that suggest that there is no cholecystitis- one thing
There is no history of jaundice or fever
Heartburns and upper abdominal pain can be due to dyspepsia or acid peptic disease along with reflux
So keeping above points in mind, if you had been my patient I would have suggested you not to go for cholecystectomy at least presently.
Continue with above medicines which have been prescribed to you and may take antispasmodic like drotavarine tablet on sos basis that is whenever pain is there
Few more tips:
1.Avoid fatty meals
2. Don’t lie down soon after meals for at least two hour
3.Take small and frequent meals, instead of full meals at a time
4. wear loose clothes
5. If you are overweight or obese do weight reduction with dieting and exercise
6. Instead of Nexprofast take Nexpro-l once in a day before breakfast. It contains levosulpiride which is a prokinetic agent that is more useful in reflux disease
I hope I have answered your query and this will help you. If you have any further query I will be happy to answer that too. Remain in touch and get-well soon.
I may have missed one point in my earlier question. I had similar heartburn/acidity problems 3 years ago over a period of 3 months right after my pregnancy. I had got endoscopy done, the reports for which come out normal. The problem subsided on its own without any medications.
This problem is recurring again now from last 1 month. I got ultrasound scans done yesterday on fasting again just to be doubly-sure. Please review and advise.
Thanks, xxxxx
No need of cholecystectomy at present

Welcome back
I have gone through the reports you have attached
This repeat ultrasound abdomen is showing almost the same findings that is gallbladder stones without thickened wall which again suggest thatthere is no cholecystitis. So there is no need of undergoing cholecystectomy presently at least.
Reflux symptoms like heartburns and acidity are very common during pregnancy and soon after pregnancy. Nice to hear that it has resolved spontaneously
Continue with the medicines advised earlier and get well soon. Wish you a good health
