Suggest Treatment For Hemophagocytic Lymphohistiocytosis

Question: My brother was suffering from HLH. During the investigation it was found that he has Thymoma B2. He was treated with steroid for HLH & then he went under surgery for Thymoma. It was removed 15 days back, but since then his body temperature is 99 - 101 degree F. There is a drastic weight loss. Also suffering from continuous sleep & restlessness.
Your suggestion & recommendation will be highly appreciated, as the Doctors of XXXXXXX Hospita, XXXXXXX XXXXXXX are unable to diagnose.

I am Dr Jyothi BL and I would like to take up your query
Looking at the history and diagnosis you have given, my opinion is as follows.
1. Fever and other symptoms might be attributed to Hemophagocytic Lymphohistiocytosis (HLH).Since immunity will be low, he will be prone for infections. Also he was on steroids for quite sometime which adds on to the brunt of already immunocompromised state.
2. Discuss with the consulting doctor regarding possible CNS involvement (can occur in HLH) for symptoms like Continuous Sleep and restlessness.
3. Thymoma B is benign, so not to bother much about it.
4.However he needs to be evaluated for any other associated diseases depending on symptoms as Thymoma is most often associated with autoimmune diseases.
If possible attach the reports, so that I can get into detailed description of the ongoing symptoms.

ESR & CRP are abnormal. Should he continue with antibiotics? He is having continuous fever. Abnormality may be the reason?ESR-83, CRP-29.4

According to CRP levels there appears to be a mild inflammation, so continue with present treatment (antibiotics) and follow up with CRP levels.
ESR changes appear late in inflammation, so its important to assess inflammation with repeat CRP levels.
