- an inflammatory disorder
- an electrolyte imbalance
- thyroid dysfunction
- adrenal gland dysfunction.
For this reason, I would recommend consulting with your attending physician for a careful physical exam and some tests:
- a resting ECG
- a chest X ray study
- some blood lab tests (complete blood count, thyroid hormone levels, PCR, sedimentation rate, blood electrolytes, fasting glucose, plasma levels of cortisol and aldosterone)
- an urine analysis
- urine levels of metanephrines for possible pheochromocytoma.

This a really low dose for hydrochlorothyazide.
I recommend increasing the dose to 25mg in the morning.
I would also advise adding another antihypertensive drug to your therapy such as ACEI (enalapril, lisinopril) or ARB class (valsartan, telmisartan, etc.).
If your blood pressure is still high a week after making these changes, the next step would be add a calcium channel blocker (amlodipine, nicardipine, etc.).
I would also advise performing the above mentioned tests in order to exclude possible secondary causes of high blood pressure.
