Suggest Treatment For High Fever In A Toddler

Question: My son who is 2.9 years old is having fever of 103 and it started about 9 hours back. Since then he has vomitted once and passed motion twice.. The motion was loose. We have given dolo 250 and fever does not seem to come down. Can you please suggest a possible medicine.

The most common cause of fever with loose motion and vomiting in this age is some viral infection.Here fever remains very high during initial 48-72 hours.Dolo contains paracetamol and the usual dose is 10-20 mg/kg/dose (maximum 60 mg/kg/day)and it can be given at an interval of 4 to 8 hours depending on the severity.
First check whether the dose is appropriate or not and it can be repeated at an interval of 4 hours.Do tepid sponging as it helps in bringing down the temperature.
If fever still remains high you can give mefenemic acid/meftal p (after consulting your physician).The dose is 6-8 mg/kg/dose(25 mg/kg/day)and can be repeated at an iterval of 8 hours.
The main aim of management in this condition is to prevent dehydration by giving plenty of fluids/ORS.Give some anti emetic like ondensetron if there is repeated vomiting.Giving probiotic and zinc helps in early recovery.
If fever is uncontrolled,vomiting is persistent,baby gets lethargic or oral acceptance is poor consult your pediatrician immediately.
