Brief Answer:
sponging and paracetamol.

Detailed Answer:
Hi, thanks for using health care magic.
Does the fever occur with the chills? Does the fever come daily?
The fever is of very high grade. But the total leucocyte count is in the normal range for his age. So most likely it is not bacterial infection.
It could be viral fever or may be malaria.
In my opinion first of all you urgently do tepid sponging ( sponging with the tap water) for him to bring his fever down. Also give him paracetamol 15 mg/kg of body weight.
Have you done the peripheral smear for the malarial parasite? If not then I would recommend you to get it done.
If the fever does not subside or if recurs then please consult a paediatrician as physical examination of the child is very important.
I hope I have helped you
If you have any further queries then please feel free to ask.
Thank you

Brief Answer:
most probably viral fever

Detailed Answer:
Hi, thanks for the follow up query. If the malarial parasite check is found to be negative and the fever is daily then most likely to be viral fever.
Usually viral fever last not more than seven days. If it persists beyond that then we may have to rule out the collagen vascular diseases as reason for such high grade fever. So if fever persists beyond seven days then please get a review done by a paediatrician.
If the fever does not subside with paracetamol then I would start the child on combiflam 5 ml. Your local doctor can prescribe combiflam schedule if needed.

I hope I have helped you
Thank you
